We have been privileged to be entrusted with your medical care over the recent past and we hope that Quality of Life Medical Center (QLMC) has met or exceeded your expectations during that time. Things change and due to no fault of her own, Susan LeBouef will no longer be practicing at Quality of Life Medical Center.
We are pleased to say that we have several colleagues at QLMC who will be pleased to take over responsibility for your care. Both Kindra Burnell, FNP and Cindy Finch-Benoy, FNP are excellent providers and either one will ensure that you receive the quality care you have come to expect. Change can be difficult. I am certain that QLMC will do whatever is necessary to ensure a smooth transition for you.
Should you for personal reasons choose to move to a different clinic, know that your medical records will be maintained at Quality of Life Medical Center. If you need copies of your records, you can find an “Authorization for Release of Information” form on the website at https://qlmc.com/patients/patient-forms/. If you are unable to access that form, simply call the office and we will make sure you get a copy of the form to complete, and we will transfer your records as requested.
